 CONFIDENTIALITY AGREEMENT
The Texas Lice Squad™ will not divulge your name or make any reference to any family member to anyone, public or private, concerning
consultations or treatment of pediculosis without your permission.
When visiting our office, if we are asked “Did you treat (insert name)” the answer will ALWAYS be “no”. We will provide the same courtesy and confidentiality if we
are asked about you or your family members.
We strive to prevent the spread of head lice and strongly encourage parents to inform others of the problem in order to prevent the proliferation of a lice infestation.
With your permission, we would be happy to contact your child’s school/daycare/camp and advise them of a possible infestation.
